Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate and treat patients in home or facility settings. You will participate in care planning, document progress, and provide patient and family instruction while maintaining professional standards and confidentiality.

The role requires a minimum six months of experience as an SLP, a current license, CPR certification, and a valid drivers license with reliable transportation.

Qualifications

• Minimum six months experience as a speech therapist / SLP.
• Current and unrestricted license.
• Current CPR certification.
• A valid drivers license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent and minimize effects.
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening.
• Observe, record and report changes in the patients condition to supervisor and/or physician.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation as required by the Company and/or facility.
• Participate in care coordination and discharge planning.
• Maintain confidentiality and share information only with authorized recipients.
