Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate and treat patients in their homes or facilities. You will develop and review treatment plans, assess disorders of speech and swallowing, and coordinate care with supervisors and physicians.

The role requires a CCC-SLP (ASHA), a valid state license, CPR certification, and a reliable vehicle. Prior home health experience is preferred.

Qualifications

CCC-SLP certification (ASHA) required.
Minimum six months experience as speech-language pathologist.
Current and unrestricted license; CPR certification.
Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

Evaluate and provide speech-language pathology services in the home or facility.
Develop and review Plan of Treatment and Plan of Care.
Assess and treat disorders of speech, voice, language, hearing and swallowing.
Administer and interpret diagnostic tests and therapeutic treatments.
Observe and report changes to supervisor/physician.
Provide instruction on alternative communication systems when appropriate.
Counsel patients, families and healthcare staff.
Maintain documentation; prepare progress reports and care plans.
Participate in care coordination and discharge planning.
