Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to deliver in-home care focused on rehabilitation and functional improvement. You will evaluate, treat, and coordinate care for patients with speech, language, voice, hearing, and swallowing disorders in their homes or facilities.

Qualifications

• CCC-SLP or Audiology certification required.
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Evaluate and provide speech-language pathology services in home or facility.
• Develop and review Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapeutic applications.
• Maintain documentation and progress reports as required.
