FOX Rehabilitation is expanding in Bartlett, TN, seeking licensed Speech Language Pathologists to deliver one-on-one care in homes and senior living communities. You’ll evaluate, plan, and treat patients, documenting in the EMR and collaborating with PT/OT/SLP teams to promote aging in place.

Qualifikationen

• Valid Speech Language Pathology license in the state(s) of practice or eligibility.
• Degree from an accredited Speech Language Pathology program and CCC certification.
• Proficiency with EMR systems and CPR certification.

Aufgaben

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living settings.
• Perform evaluations, develop individualized care plans, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and independence.
• Document effectively in the EMR per regulatory standards.
• Collaborate with an interdisciplinary team and educate patients, caregivers, and staff.
• Provide telehealth when appropriate to extend access.
