Fox Rehabilitation is seeking a licensed Speech Language Pathologist to deliver one-on-one care in older adults' homes or Senior Living Communities in Bartlett, TN. You will conduct evaluations, create care plans, and deliver evidence-based therapy to improve mobility, independence, and quality of life.

The role offers autonomy with flexible scheduling, the option to incorporate telehealth, and collaboration with a multidisciplinary team to support aging in place.

Formación

• Licensed Speech Language Pathologist in the state(s) of practice or eligibility to obtain license.
• Degree from an accredited Speech Language Pathology program.
• Current CPR certification is required.
• Current ASHA CCC certification is required.

Responsabilidades

• Provide one-to-one Speech Language Pathology to older adults in homes or senior living settings.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to support wellness, mobility, and independence.
• Document in FOX's EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
