Job summary

Fox Rehabilitation is hiring a Speech Language Pathologist for Spring Hill, FL, delivering in-home and senior living care with opportunities for telehealth. The role emphasizes autonomy, flexible scheduling, and meaningful patient outcomes, serving older adults in private residences and communities.

Responsibilities include evaluations, care planning, and one-on-one treatment, with documentation in the EMR and collaboration with an interdisciplinary team.

Qualifications

• EMR proficiency and basic computer skills.
• CPR certification is current.
• CCC ASHA credential is current.

Responsibilities

• Provide one-on-one Speech Language Pathology services to older adults in homes or community settings.
• Perform evaluations, develop individualized care plans, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and independence.
• Document in FOX’s EMR per regulatory standards.
• Collaborate with an interdisciplinary team.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
