Job summary

Qualifications

• Licensed Speech-Language Pathologist in the state of practice.
• Experience delivering one-on-one in-home or senior living care preferred.
• Ability to develop and implement evidence-based treatment plans.

Responsibilities

• Provide one-on-one speech language pathology to older adults in homes or senior living communities.
• Perform evaluations, develop individualized care plans, and deliver evidenced-based treatment.
• Deliver sessions that promote wellness, mobility, and independence.
• Document in EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.
• Provide telehealth when appropriate to extend access.
