Job summary

Fox Rehabilitation in Riverdale, NY is seeking licensed Speech Language Pathologists to deliver one-on-one geriatric care in private residences and senior living communities. The role offers autonomy to create your own schedule and may include telehealth, with a focus on improving mobility, independence, and quality of life.

Responsibilities include assessments, individualized plans of care, and documenting in FOX’s EMR.

Qualifications

• Valid SLP license in the state(s) of practice.
• Degree from an accredited Speech Language Pathology program.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC).
• Basic EMR proficiency.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living communities.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to support wellness, mobility and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
