Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to deliver one-on-one geriatric care in patients' homes or senior living communities across Paramus, NJ and surrounding areas. The role offers autonomy, flexible scheduling, and the chance to build lasting patient relationships using the Geriatric House Calls model.

You'll perform evaluations, create plans of care, deliver evidence-based therapy, document in our EMR, and collaborate with an interdisciplinary team.

Qualifications

• Requires active SLP license in NJ or eligibility to obtain
• CPR certification current
• Proficiency with EMR systems and basic computer use

Responsibilities

• Provide one-on-one speech-language pathology services to older adults in-home or in senior living communities
• Perform evaluations and develop individualized plans of care
• Deliver evidence-based therapy to improve communication, safety and independence
• Document in EMR per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote aging in place
• Offer telehealth/virtual care when appropriate
