Job summary

FOX Rehabilitation seeks a licensed Speech Language Pathologist for in-home and senior living care in Chestnut Ridge, NY. You will evaluate, plan, and deliver evidence-based therapy tailored to older adults, with scheduling flexibility and potential telehealth options to extend reach.

You will collaborate with an interdisciplinary team and document in FOX’s EMR, supporting quality outcomes and independent living for clients.

Qualifications

• Must hold valid NY SLP license or be eligible to obtain it.
• Degree from an accredited Speech Language Pathology program.
• Current CPR certification.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in home or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and functional independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
