Job summary

Fox Rehabilitation is seeking a licensed Speech Language Pathologist in Chestnut Ridge, NY to deliver one-on-one care to older adults in homes and senior living communities.

You’ll develop individualized care plans, perform evaluations, and provide evidence-based therapy while documenting in a compliant EMR. Telehealth options may be used to extend access and coordination with an interdisciplinary team.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Speech Language Pathology program.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC)

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in home or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions supporting wellness, mobility, and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.

Education

Degree from an accredited Speech Language Pathology program

ASHA CCC (Certificate of Clinical Competence)

SLP licensure in NY or eligible
