Job summary

FOX Rehabilitation is hiring a Speech Language Pathologist for Riverdale, NY to deliver one-on-one care in home and senior living settings. You will evaluate patients, create individualized plans, and provide evidence-based therapy to improve mobility and independence.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice.
• Degree from an accredited Speech Language Pathology program.
• CPR certification.
• Current ASHA CCC (Certificate of Clinical Competence).
• Basic EMR and computer proficiency.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in private residences or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and independence.
• Document effectively within FOX's EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/ virtual care when appropriate to extend access.
