Job summary

FOX Rehabilitation in Yonkers, NY is seeking a licensed Speech Language Pathologist to deliver one-on-one care in homes and senior living communities as part of the Geriatric House Calls model.

You will evaluate, plan, and provide evidence-based therapy while building strong patient relationships and documenting in the EMR. Telehealth may be incorporated to extend access and flexibility.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain.
• Current Certificate of Clinical Competence (ASHA CCC) is preferred.
• Degree from an accredited Speech Language Pathology program.
• CPR certification is required.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living settings.
• Develop individualized plans of care and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, longevity, and independence.
• Document in FOX's EMR per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
