Pinnacle Home Care in Florida is seeking a Speech Language Pathologist (ST/SLP) to provide in-home services on a PRN basis. You will assess, plan, implement, and monitor therapy for patients in their homes, collaborating with physicians and an interdisciplinary team.

The role requires Florida licensure, a BLS certification, and a minimum of two years of clinical experience, with strong communication and independent work capabilities. A valid driver’s license and reliable vehicle are required.

Formación

• Graduate of an accredited Speech Therapy program.
• Licensed Speech Therapist in Florida.
• Minimum two years clinical experience as a Speech Therapist.
• Current BLS Certification.

Responsabilidades

• Evaluate clients’ speech, language, and hearing abilities and propose strategies.
• Perform initial and ongoing assessments, including OASIS evaluations.
• Develop and revise treatment plans with physician input.
• Collaborate with interdisciplinary team on care plans and train team members.
• Educate clients and families on therapy techniques.
• Document evaluation data, treatments, and progress promptly.
• Supervise Home Health Aides every 14 days when Speech Therapy is the sole discipline.
• Communicate with physicians to ensure appropriate orders.
• Update care plans and obtain additional orders as required.
