Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to provide in-home and senior living care in Woodbridge, NJ. You will conduct evaluations, develop plans of care, and deliver evidence-based therapy to older adults in private residences and communities.

You will enjoy autonomy over your schedule, opportunities for telehealth, and collaboration with an interdisciplinary team dedicated to improving mobility and quality of life.

Qualifications

• Valid Speech Language Pathology license in the state of practice or eligibility to obtain license.
• Degree from an accredited Speech Language Pathology program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in private residences or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and independence.
• Document effectively within FOX's EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.
