Job summary

FOX Rehabilitation is hiring a Speech Language Pathologist to provide in-home and senior living care in Piscataway, NJ. You will evaluate patients, create individualized care plans, and deliver evidence-based therapy to help older adults improve communication, safety and independence.

Hybrid flexibility with telehealth options and a collaborative interdisciplinary team will support clinicians in delivering high-impact care and meaningful patient outcomes.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Speech Language Pathology program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC).

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living settings.
• Perform evaluations, develop care plans, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, longevity, mobility and independence.
• Document effectively in FOX’s EMR system in line with regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
