Creative Solutions Services, LLC in Rutland, Vermont is seeking a Speech Language Pathologist (SLP) for an in-home/outpatient rehabilitation assignment. The 17-week contract offers a fixed schedule: 8:00 AM to 4:30 PM, 40 hours per week, onsite in Rutland.

Qualifications

• Must have a valid Speech Language Pathologist license in the state(s) of practice.
• Must possess or be eligible for ASHA CCC from the American Speech-Language-Hearing Association (ASHA) or be eligible to participate as an ASHA Clinical Fellow.
• Must have a valid driver's license or reliable transportation to patients' residences.

Responsibilities

• Supervise CFYs, temporary licensees, students and support personnel as needed.
• Adhere to patient treatment schedules and complete clinical and billing documentation.
• Collaborate with rehabilitation team and physician to select appropriate therapy regimens.
• Coordinate care in the home and community with interdisciplinary team members.
