Job summary

CenterWell Home Health is seeking a Speech-Language Pathologist to evaluate and provide speech-language pathology services to patients in home or facility settings. You will participate in developing and reviewing the Plan of Treatment and Plan of Care, diagnose and treat disorders, and maintain documentation.

You must have ASHA certification, at least six months of experience, a current license and CPR, plus a valid driver's license and reliable transportation.

Qualifications

• Meet the education and experience requirements for Certification of Clinical Competence in Speech-Language Pathology or Audiology (ASHA).
• Minimum six months experience as a speech therapist / speech-language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation are required.
• Preferred: Home Health experience.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing; perform diagnostic tests and interpret results.
• Observe, record and report changes in patient condition to supervisor/physician.
• Provide training to patients in use of alternative communication systems when appropriate.
• Maintain documentation and submit reports as required by policy and for care coordination.
