Job summary

Apollo Home Healthcare is seeking a Speech-Language Pathologist (SLP) to provide in-home visits across Contra Costa, Alameda, and Tri-Valley areas in California. This W-2 per diem role offers schedule flexibility, autonomy, and meaningful patient relationships within a supportive interdisciplinary team.

As a Home Health SLP, you will evaluate, plan, and provide one-on-one therapy in patients' homes, collaborating with physicians and care teams, and completing compliant documentation.

Qualifications

• Active California SLP license
• Master's degree in Speech-Language Pathology
• CPR certification
• Home health experience preferred
• Strong documentation and communication skills
• Ability to work independently in the field

Responsibilities

• Perform speech-language and swallowing evaluations in patients' homes
• Develop and implement individualized Plans of Care
• Provide skilled speech therapy interventions
• Recommend communication devices and adaptive equipment as needed
• Assess home environments for safety and functional communication needs
• Collaborate with physicians, nurses, PT/OT, and care teams
• Participate in case conferences as appropriate
• Complete timely, compliant clinical documentation
• Educate patients and caregivers on exercises and compensatory strategies
• Supervise home health aides when applicable to the plan of care
