Job summary

CVHCare, a leader in Home Health Clinical services, seeks a Speech and Language Pathologist to join our California home health team in San Ramon and surrounding Vallejo area. This per diem role offers flexible scheduling and the autonomy to manage a patient-centered caseload.

The SLP will diagnose, treat and prevent communication, voice and swallowing disorders, recording progress in the EMR and collaborating with care teams. California licensure and CFY completion preferred.

Qualifications

• Valid CA SLP license in good standing.
• Open to entry level or CFY completion.
• Reliable transportation and auto insurance.
• CPR/BLS certification.
• Ability to complete EMR documentation timely.

Responsibilities

• Diagnose, treat and prevent speech, voice and swallowing disorders.
• Record and analyze speech, language and swallowing irregularities.
• Help patients develop or recover communication skills.
• Participate in case conferences and care team meetings remotely.
• Maintain schedules and coordinate home visits.
