Apollo Home Healthcare is seeking a Speech-Language Pathologist (SLP) to provide in-home therapy across Contra Costa, Alameda, and Tri-Valley areas. This W-2 per diem role offers schedule flexibility, autonomy, and meaningful patient relationships in a home-based setting.

You will work with an interdisciplinary team to deliver high-quality, one-on-one care to patients in their homes while maintaining strong documentation and professional collaboration.

Qualifications

• Active California Speech-Language Pathologist license.
• Master's degree in Speech-Language Pathology.
• Strong documentation and communication skills.

Responsibilities

• Perform speech-language and swallowing evaluations in patients' homes.
• Develop and implement individualized Plans of Care.
• Provide skilled speech therapy interventions.
• Recommend communication devices and adaptive equipment as needed.
• Assess home environments for safety and functional communication needs.
• Collaborate with physicians, nurses, PT/OT, and care teams.
• Participate in case conferences as appropriate.
• Complete timely, compliant clinical documentation.
• Educate patients and caregivers on exercises and compensatory strategies.
• Supervise home health aides when applicable to the plan of care.

What You'll Do

As a Home Health SLP, you will:

Requirements
