Apollo Home Healthcare is hiring a Speech-Language Pathologist (SLP) to provide in-home speech therapy visits across Contra Costa, Alameda, and Tri-Valley areas. This W-2 per diem role offers flexible schedule, autonomy, and meaningful patient relationships within a cooperative team.

You will assess, diagnose, and treat patients in their homes, develop plans of care, and collaborate with a multidisciplinary team to ensure quality care and compliant documentation.

Qualifications

• Active California Speech-Language Pathologist license.
• Master's degree in Speech-Language Pathology.
• Current CPR certification.
• Home health experience preferred but not required.
• Strong documentation and communication skills.
• Ability to work independently in the field.
• Reliable transportation and valid driver's license.

Responsibilities

• Perform speech-language and swallowing evaluations in patients' homes.
• Develop and implement individualized Plans of Care.
• Provide skilled speech therapy interventions.
• Recommend communication devices and adaptive equipment as needed.
• Assess home environments for safety and functional communication needs.
• Collaborate with physicians, nurses, PT/OT, and care teams.
• Participate in case conferences as appropriate.
• Complete timely, compliant clinical documentation.
• Educate patients and caregivers on exercises and compensatory strategies.
• Supervise home health aides when applicable to the plan of care.
