Job summary

FOX Rehabilitation in Hunterdon, NJ seeks a licensed Speech Language Pathologist to deliver one-on-one care in private residences and senior living communities. This role offers autonomy, flexible scheduling, and the opportunity to build lasting patient relationships within our Geriatric House Calls model.

You may also provide telehealth or virtual care, participate in an interdisciplinary team, complete evaluations, develop plans of care, document in the EMR, and educate patients and caregivers

Qualifications

• Licensed Speech Language Pathologist in state(s) of practice or eligible to obtain
• Degree from an accredited Speech Language Pathology program
• CPR certification up to date and Emr proficiency
• Current Certificate of Clinical Competence (ASHA CCC)

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in private residences or senior living communities
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions to support wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility
