Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to provide one-on-one care in Jersey City and surrounding NJ communities, delivering services in private residences and senior living settings under the Geriatric House Calls model.

You will conduct evaluations, develop individualized plans, deliver therapy to improve communication, mobility, and independence, document in our EMR, and collaborate with PTs, OTs, SLPs, and Fitness Specialists; telehealth may be included to extend

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain a degree from an accredited program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC).

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in private residences or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support patient wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.
