Job summary

FOX Rehabilitation is expanding in Upper West Side, NY, and seeks licensed Speech Language Pathologists dedicated to delivering high-quality, in-home and senior living care. Our Geriatric House Calls™ model enables you to treat patients where they live, with autonomy over your schedule and meaningful outcomes.

You'll work in private residences or Senior Living Communities, with optional telehealth to extend access and continuity of care.

Qualifications

• Valid Speech Language Pathology license in the state of practice or eligibility to obtain.
• Current CCC from ASHA and CPR certification are required.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in private residences or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, longevity, mobility, and independence.
• Document effectively in FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
