FOX Rehabilitation is seeking licensed physical therapists to deliver in-home and senior-living-based geriatric care in Beaverton, OR. You will design and implement one-on-one treatment plans, improve mobility and independence, and coordinate with a multidisciplinary team to optimize patient outcomes.

Clinicians may work across home and community settings, with options for telehealth and flexible scheduling; in-home care requires driving to patient residences and facilities in the Beaverton

Qualifikationen

• Licensed Physical Therapist in the relevant state or eligibility to obtain license.
• Degree from an accredited Physical Therapy program (DPT preferred).
• Basic computer and EMR proficiency.

Aufgaben

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Deliver therapy sessions that promote mobility, independence and functional health.
• Document thoroughly within FOX’s EMR system in compliance with regulations.
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs and Fitness Specialists.
• Incorporate telehealth/virtual care when appropriate to extend access.
