Job summary

FOX Rehabilitation is expanding in Beaverton, OR, seeking licensed Physical Therapists to deliver in-home and senior living care. You will provide one-on-one PT to older adults, including private residences and living communities, while building strong patient relationships and autonomy in scheduling.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Deliver therapy sessions to support mobility, independence, and functional goals.
• Document care in FOX’s EMR system in compliance with regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Offer telehealth/virtual care when appropriate to extend access.
