Meals on Wheels San Francisco is seeking an Assessment Specialist to conduct in-home assessments for homebound seniors, determining eligibility and service needs in San Francisco. You will maintain confidential records and coordinate referrals with community resources to ensure integrated client care.

The role requires 2+ years with seniors, a bachelor's in social work or gerontology, and strong communication and organizational skills; bilingual abilities are a plus.

Qualificações

• Two+ years of hands-on experience working with seniors in San Francisco.
• Bachelor’s in social work or gerontology preferred.
• Ability to work with a diverse community.
• Strong written and verbal communication and organizational skills.

Responsabilidades

• Conduct in-home initial assessments weekly and annual assessments to determine eligibility and service needs.
• Maintain confidential electronic files and timely documentation.
• Establish records, prepare reports, and enter client data into databases.
• Coordinate referrals to community resources and ensure coordinated services.
• Support home-delivered meal operations and assist with reassessments as needed.
• Provide orientations and participate in department meetings; respond to client inquiries.
