Job summary

All Seniors Foundation is seeking a Home Health Social Worker to assist seniors in accessing essential services, provide emotional support, and guide families through healthcare, financial, and social resource navigation. This role emphasizes holistic elderly care, improving quality of life and family peace of mind.

The ideal candidate holds an MSW with a California license (LCSW preferred) and experience with geriatric populations or home health services.

Qualifications

Responsibilities

• Conduct psychosocial assessments to identify clients’ needs and challenges.

• Offer emotional support, counseling, and advocacy to help clients cope with life changes.
• Collaborate with healthcare professionals, caregivers, and external organizations.
• Maintain thorough documentation of client interactions and progress.
