Job summary

Uplift HomeCare, LLC is hiring a Registered Nurse (RN) to provide in-home health care to patients in Gibsonton, FL and surrounding areas. PRN / Part-Time with flexible scheduling, competitive compensation, and a supportive clinical team.

Qualifications

• Active Florida RN license required.
• Home health experience preferred; may vary by assignment.
• OASIS experience strongly preferred.

Responsibilities

• Perform skilled nursing assessments and evaluate patient health status.
• Develop, implement, and update individualized plans of care.
• Provide nursing treatments and monitor patients for changes in condition.
• Administer medications and treatments as ordered and within the RN’s scope of practice.
• Educate patients and caregivers on diagnoses, medications, safety, and disease management.
• Coordinate care with physicians, therapists, and other members of the healthcare team.
• Complete accurate, timely clinical documentation, including OASIS assessments when applicable.
• Follow applicable state and federal regulations, infection-control practices, HIPAA requirements, and agency policies.
