Qualifications

• Ability to lift 25 lbs and drive to client homes.

Responsibilities

• Travel to assigned patient homes and manage care per physician instructions.
• Assess and document patient condition at each visit.
• Review medications and vital signs; perform wound care as needed.
• Educate caregivers and family on aftercare and ongoing care.
• Provide palliative care to keep patient comfortable.
• Recommend devices to improve quality of life.
