Job summary

Uplift HomeCare LLC in Tampa, FL is seeking a compassionate Registered Nurse (RN) to join our home health team serving Tampa and surrounding communities. The PRN/part-time role offers flexible scheduling and competitive compensation to provide one-on-one, patient-centered care in patients’ homes.

The RN will perform assessments, develop plans of care with the physician and care team, administer medications, educate patients and families, and maintain accurate EMR documentation.

Qualifications

• Current, unrestricted Florida RN license.
• Home health nursing experience preferred.
• OASIS experience strongly preferred.
• Experience with electronic medical records; Axxess experience is a plus.
• AHCA Level II background screening.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and current auto insurance.
• Strong clinical assessment, critical‑thinking, and documentation skills.
• Excellent written and verbal communication.
• Ability to work independently while maintaining communication with the clinical team.

Responsibilities

• Provide skilled nursing care in the home and coordinate services.
• Perform initial, comprehensive, and ongoing nursing assessments.
• Develop, implement, and evaluate individualized plans of care with the physician and team.
• Administer medications and treatments per orders.
• Monitor vital signs and patient condition, and escalate changes as needed.
• Educate patients and caregivers on medications, disease management, and home safety.
• Coordinate care with physicians, therapists, case managers, and other professionals.
• Complete accurate clinical documentation in the EMR system.
• Complete OASIS documentation when applicable.
• Maintain HIPAA compliance and adhere to policies and regulations.
• Participate in training, in-services, and quality improvement activities.
• Deliver compassionate, patient-centered care promoting dignity, safety, and independence.
