Job summary

AdventHealth in Temple Terrace, FL is hiring a registered nurse for home health case-management. The role requires evaluating patient homes, coordinating care, and delivering skilled nursing with attention to safety and infection control.

You will work Monday–Friday, 8am–5pm with on-call rotations, driving to patient homes across Hillsborough and surrounding counties. An active FL RN license and BLS are required, with 1+ years of home health experience preferred.

Qualifications

1+ relevant clinical nursing experience.
Experience as a case-manager in a Medicare-certified home health agency.
Current RN license and BLS certification required.

Responsibilities

Evaluate home environments for safety, infection control, and community resources.
Review patient history, exam results, and labs; report abnormal results to physician.
Provide skilled nursing care and prescribed treatments in home settings.
Use health coaching and motivational interviewing to engage patients and families.
Maintain clinical records and update care plans and certifications.
Optimize daily schedules to improve productivity and visit utilization.
Develop patient-specific care plans with patient, family, and physician.
