Alpha Home Health and Hospice is seeking a Registered Nurse (RN) for our PACE program in the Boston area. The role focuses on delivering compassionate, high-quality nursing care within an all-inclusive care model that supports patients in their homes.

You will collaborate with an interdisciplinary team to develop and implement individualized care plans, educate patients and families on care, safety, and health management, and ensure accurate documentation in line with clinical standards.

Qualifications

• Current RN licensure.
• Strong clinical assessment and critical thinking skills.
• Ability to work collaboratively in a team-based care environment.
• Excellent communication, empathy, and patient-centered approach.
• Sound judgment, attention to detail, and commitment to quality care.

Responsabilités

• Assess patient needs and monitor health status
• Develop, implement, and evaluate individualized nursing care plans
• Administer medications and provide skilled nursing interventions
• Coordinate with clinicians and service teams to support comprehensive care
• Educate patients and families on care, safety, and health management
• Maintain accurate documentation and ensure adherence to clinical standards
