Job summary

Alpha Home Health and Hospice is seeking an RN Case Manager I to coordinate comprehensive, person-centered nursing care for frail seniors enrolled in a PACE program in Lynn, MA. The role emphasizes safety, independence at home, and collaboration with families and an interdisciplinary team.

You will conduct assessments, develop care plans, educate participants and families, and advocate for access to resources to maintain quality of life in the community setting.

Qualifications

• Registered Nurse (RN) with strong clinical assessment and care coordination skills.
• Ability to develop and implement care plans in collaboration with a care team.
• Excellent communication and patient/family education skills.
• Strong advocacy mindset and commitment to safe, independent living.
• Comfort working in community-based and clinic environments.

Responsibilities

• Conduct nursing assessments and develop individualized care plans
