Job summary

CareSource is seeking a Clinical Care Manager in Massachusetts to provide in-home and community-based care management for dually-eligible enrollees with complex medical and social needs. The Nurse will coordinate care across providers, assess needs, and develop individualized care plans.

Responsibilities include leading the interdisciplinary team, promoting access to services, educating members about benefits, and supporting transitions of care to improve outcomes and quality of life.

Qualifications

• ASN in nursing or MSW/LMHC with independent license required.
• 3 years of RN/BH clinician experience or 1 year with 2 years in complex medical/behavioral/social needs as LPN/CHW/MA required.
• Prior experience in care coordination or working with dual-eligible populations preferred.

Responsibilities

• Engage enrollees in their homes and community settings to establish a complex care management relationship.
• Liaise between healthcare providers, community resources, and enrollees for seamless care transitions.
• Perform comprehensive assessments and functional assessments including MDS-HC.
• Lead care plan development and update on status changes.
• Coordinate with ICT to create holistic care plans addressing medical and non-medical needs.
• Oversee enrollee utilization of long-term services to help stay in preferred location.
