Job summary

ComForCare in Fort Collins, CO is seeking an experienced Registered Nurse to lead in-home care for a caseload of clients. You will assess needs, develop care plans, supervise caregivers, and coordinate with families and physicians to ensure compassionate, compliant home health services.

Requires active Colorado RN license, 1+ year home care experience, BLS/CPR, valid driver’s license, and Oasis documentation proficiency. This FT role offers a daytime schedule and benefits.

Qualifications

• Active, unencumbered RN license in Colorado.
• ≥1 year home care/home health nursing experience.
• Case management or caregiver supervision experience preferred.
• Current BLS/CPR certification.
• Valid driver’s license and reliable transportation.
• Oasis Documentation.
• Strong clinical judgment and communication skills.

Responsibilities

• Complete initial and ongoing in-home nursing assessments.
• Create and update plans of care with caregivers.
• Oversee a caseload and adjust services as needs change.
• Supervise and train caregivers; conduct competency checks.
• Coordinate care with families, physicians, and hospitals.
• Maintain accurate records per Colorado home care regulations and HIPAA.
• Provide on-call support for client and caregiver concerns.
