Job summary

ComForCare Home Health Care - Northern CO in Fort Collins, CO is seeking an experienced Registered Nurse in Home Care to lead client caseloads and supervise caregivers in a private home setting.

Responsibilities

• Care coordination: act as main clinical contact for families and physicians; make referrals
• Documentation and compliance: maintain accurate records; follow regulations and HIPAA

Registered Nurse In Home Care | Fort Collins, Colorado

Pay: $47 – $52 per hour, based on experience

Schedule: Monday – Friday day shifts (8 a.m. – 5 p.m.), flexible scheduling, occasional holidays, weekend rotation

What You’ll Do as an RN Case Manager

• Client intake and assessments: Complete initial and ongoing in-home nursing assessments of each client’s physical, cognitive, psychosocial and home safety needs
• Care plan development: Create and update individualized plans of care and make sure caregivers understand and follow them
• Caseload management: Oversee a caseload of home care clients, tracking changes in condition and adjusting services as needs change
• Caregiver supervision and training: Conduct supervisory visits and complete competency checks
• Care coordination: Serve as the main clinical contact for families, physicians, hospitals and community partners; make referrals when needed
• Documentation and compliance: Keep accurate, timely client records and follow Colorado home care regulations, infection control and HIPAA guidelines
• On-call support: Help resolve client and caregiver concerns quickly and professionally

Qualifications

• Active, unencumbered Registered Nurse (RN) license in Colorado
• At least 1 year of home care or home health nursing experience (required)
• Case management, care coordination or caregiver supervision experience (preferred)
• Current BLS/CPR certification
• Valid driver’s license and reliable transportation
