ComForCare Home Health Care - Northern CO in Greeley, Colorado is seeking an experienced Registered Nurse to lead in-home care, manage client caseloads, guide caregivers and build lasting client-family relationships.

Responsibilities include in-home assessments, care plan development, caseload oversight, caregiver supervision, clinical coordination with families and providers, and ensuring compliant documentation and on-call support.

Qualifications

• Active, unencumbered RN license in Colorado.
• Minimum 1 year in home care/home health nursing.
• Current BLS/CPR certification.
• Valid driver’s license and reliable transportation.
• Oasis Documentation.

Responsibilities

• Client intake and assessments: initial and ongoing in-home nursing assessments.
• Care plan development: create and update individualized plans of care.
• Caseload management: oversee home care clients and adjust services.
• Caregiver supervision and training: conduct supervisory visits and competency checks.
• Care coordination: act as clinical liaison for families, physicians, and hospitals.
• Documentation and compliance: maintain records per regulations and HIPAA.
• On-call support: resolve client and caregiver concerns promptly.
