Job summary

ComForCare in Fort Collins, Colorado is hiring an experienced RN Case Manager for home care. You will manage your own client caseload, guide a team of caregivers and build lasting relationships with clients and families, with a schedule Monday–Friday 8 a.m.–5 p.m.

Qualifications

• Active, unencumbered RN license in Colorado.
• At least 1 year of home care or home health nursing experience.
• Case management or caregiver supervision experience preferred.
• Current BLS/CPR certification.
• Valid driver's license and reliable transportation.
• HIPAA compliance knowledge.
• Strong clinical judgment and communication skills.

Responsibilities

• Client intake and assessments: Complete initial and ongoing in-home nursing assessments of each client’s physical, cognitive, psychosocial and home safety needs.
• Care plan development: Create and update individualized plans of care and ensure caregivers follow them.
• Caseload management: Oversee a caseload of home care clients and adjust services as needs change.
• Caregiver supervision and training: Conduct supervisory visits and competency checks.
• Care coordination: Serve as the main clinical contact for families, physicians, hospitals and partners; make referrals as needed.
• Documentation and compliance: Maintain accurate client records and follow regulations (HIPAA).
• On-call support: Help resolve client and caregiver concerns quickly and professionally.

Registered Nurse Case Manager – Home Care | Fort Collins, Colorado

Pay: $47 – $52 per hour, based on experience

Schedule: Monday – Friday day shifts (8 a.m. – 5 p.m.), flexible scheduling, occasional holidays, weekend rotation

Qualifications
