ComForCare is seeking a Registered Nurse in Greeley, Colorado to lead in-home care caseloads and mentor caregivers. You’ll manage client plans, assess needs, and coordinate with families and providers in a patient-centered environment.
The ideal candidate holds an active Colorado RN license, at least 1 year of home care experience, and BLS/CPR certification. This full-time, on-site role offers a flexible schedule and competitive benefits.
Competenze
- Active, unencumbered RN license in Colorado
- At least 1 year of home care experience
- Case management or caregiver supervision experience preferred
- Current BLS/CPR certification
- Valid driver's license and reliable transportation
- Oasis Documentation experience
- Strong clinical judgment and communication skills
Mansioni
- Client intake and assessments of physical, cognitive, psychosocial needs
- Care plan development and updates for each client
- Oversee a caseload of home care clients and adjust services
- Supervise and train caregivers and complete competency checks
- Coordinate care with families, physicians and hospitals
- Maintain accurate client records and comply with regulations
- Provide on-call support for clients and caregivers
