Job summary

ComForCare is seeking an experienced RN Case Manager to operate in Fort Collins and surrounding areas. You will manage your own client caseload, guide a team of caregivers, and build lasting relationships with clients and families, delivering compassionate, in-home care.

Qualifications

• Active, unencumbered RN license in Colorado.
• At least 1 year of home care or home health nursing experience.
• Case management, care coordination or caregiver supervision experience.
• Current BLS/CPR certification.
• Oasis Documentation.
• Valid driver’s license and reliable transportation.

Responsibilities

• Client intake and assessments: Complete initial and ongoing in-home nursing assessments of each client’s needs.
• Care plan development: Create and update individualized plans of care and ensure caregivers follow them.
• Caseload management: Oversee a caseload of home care clients and adjust services as needs change.
• Caregiver supervision and training: Conduct supervisory visits and competency checks.
• Care coordination: Serve as main clinical contact for families, physicians, hospitals, and partners; make referrals when needed.
• Documentation and compliance: Keep accurate client records and follow Colorado home care regulations, infection control and HIPAA guidelines.
• On-call support: Help resolve client and caregiver concerns quickly and professionally.

• Caregiver supervision and training: Conduct supervisory visits and competency checks
• Care coordination: Serve as the main clinical contact for families, physicians, hospitals and community partners; make referrals when needed
• Documentation and compliance: Keep accurate, timely client records and follow Colorado home care regulations, infection control and HIPAA guidelines
• On-call support: Help resolve client and caregiver concerns quickly and professionally

Qualifications
