Job summary

Traditional Home Health Care is seeking an RN Case Manager to join our team in Pennsylvania. This role focuses on delivering high-quality, one-on-one home health care with collaboration among physicians, nurses, and families.

The position offers full-time hours from Monday to Friday and includes supporting patient care in the home setting. You will assess patients, develop plans, administer medications, document care, and coordinate with therapists to ensure comprehensive services.

Qualifications

• Active RN license in PA or eligible to practice.
• BLS certification is required.
• Strong assessment and documentation skills.
• Excellent communication and interpersonal skills.
• Experience in acute care or home health preferred.

Responsibilities

• Assess patients and develop personalized care plans with physicians, nurses, patients, and families.
• Provide hands-on care, administer medications, and evaluate patient progress.
• Maintain and submit timely documentation, including OASIS assessments.
• Collaborate with therapists and healthcare professionals to ensure comprehensive care.
• Educate patients and families on treatment plans and self-care.
• Inform patients of additional services and resources as needed.

Schedule: Full-Time -Monday through Friday 8:30am to 5:00pm

Your Role as an RN Case Manager:

What You Need to Succeed:

• Excellent Communication & Interpersonal Skills
• Acute Care and/or Home Health Experience (Preferred)
