Job summary

Professional Case Management in Ohio is seeking RN-Case Managers to conduct in-home nursing visits for former Nuclear Weapons Workers with chronic illnesses. You will provide direct care and case management, assessing patients, managing medications, and coordinating with regional staff to ensure quality home care.

Applicants must hold an RN license in the state, prefer BSN, have at least two years of nursing experience (including one in home care), and CPR certification.

Qualifications

• Graduate of a state approved school of professional registered nursing.
• Current, unrestricted RN license in the state of practice.
• Minimum of two (2) years nursing experience including one (1) year in home care or closely related field.

Responsibilities

• Provide direct care and case management of assigned clients.
• Assist and collaborate with the regional director and other personnel to identify and correct issues and/or improve services.
• Plan, implement, and evaluate care provided.
• Coordinate and manage client care conferences as needed.
• Serve as a local on-site clinical resource and support client home care needs.
• Supervise staffing and scheduling of personnel within scope of practice.
