Job summary

Professional Case Management is seeking RN-Case Managers to perform in-home visits for former nuclear workers in Ohio. You will conduct weekly nursing assessments, ensure home safety with medical equipment, and coordinate care with caregivers and staff.

Schedule is flexible, supporting school, travel, or family time. The role requires a valid RN license, 2+ years nursing experience (1+ year in home care), BSN preferred, and CPR certification.

Qualifications

• Must hold valid RN license in state of practice.
• BSN preferred or higher preferred.
• Minimum 2 years nursing experience, including 1 year in home care.
• Supervisory/case management experience preferred.
• Current CPR certification required.

Responsibilities

• Provide direct case management and nursing visits in clients' homes.
• Assess patients' health, vitals, and safety; coordinate care.
• Collaborate with regional director and staff to improve services.
• Lead staffing decisions and mentor clinical personnel.
