Job summary

Professional Case Management is hiring RN Case Managers in Texas to conduct in-home nursing visits for former Nuclear Weapons Workers with chronic illnesses. You’ll create your own schedule and work-life balance, with weekly visits that include vitals, care planning, and coordination with caregivers.

Role emphasizes patient relationships and meaningful impact on quality of life. Qualifications include an RN license, BSN preferred, and 2+ years in home care or related field, with supervisory/case

Qualifications

• RN license in state of practice required.
• BSN preferred.
• Minimum of 2 years home care or closely related field.
• 1 year of supervisory and/or case management experience preferred.

Responsibilities

• Provide direct care and case management for assigned clients.
• Collaborate with regional director to identify and correct issues and improve services.
• Plan, implement, and evaluate care provided; coordinate and manage client care conferences as needed.
• Serve as a local on-site clinical resource and provide support to ensure client home care needs are met.
• Assist with staffing coordination and scheduling within scope of practice and client needs.
• Adhere to nursing delegation guidelines and Agency policies.
• Participate in performance improvement initiatives and related documentation.
