Job summary

Professional Case Management is hiring RN-Case Managers in Washington to provide in-home nursing visits for former Nuclear Weapons Workers with chronic illnesses. You will assess patients, manage care plans, and collaborate with a regional director to enhance services.

The role emphasizes one-to-one patient relationships, a flexible schedule, and weekly visits typically lasting 1–2 hours. RN licensure and CPR certification are required.

Qualifications

• Graduate of a state approved school of professional registered nursing.
• Current, unrestricted RN license in the state(s) of practice.
• Minimum of two (2) years nursing experience including one (1) year in home care or closely related field.
• One (1) year of supervisory and/or case management experience preferred.
• Current CPR certification.

Responsibilities

• Provide direct care and case management of assigned clients.
• Plan, implement, and evaluate care provided.
• Assist and collaborate with the regional director and other personnel to identify and correct issues and/or improve services.
• Participate in client care conferences as needed.
• Serve as a local on-site clinical resource and provide support to ensure client’s home care needs are met.
• Assist staffing coordinators regarding scheduling within scope of practice and client needs.
