Job summary

Professional Case Management is seeking an RN Case Manager in Indiana to conduct in-home nursing visits for former Nuclear Weapons Workers with chronic/terminal illnesses. Visits are weekly, 1–2 hours, including vitals and home safety checks; oversee caregivers and prepare weekly medication planners.

You must hold an active RN license, with BSN preferred, and have 2+ years nursing experience including 1 year in home care; supervisory/case management experience is a plus.

Qualifications

• RN license required in practicing state(s)
• BSN preferred
• Minimum two years nursing experience including one year in home care
• One year supervisory/case management experience preferred
• CPR certification must be current

Responsibilities

• Provide direct care and case management of assigned clients.
• Coordinate with regional director and staff to improve services.
• Plan, implement, and evaluate care provided.
• Lead client care conferences as needed.
• Conduct supervisory visits to facilitate problem resolution.
• Assist staffing coordinators with scheduling within scope of practice.
