Job summary

Professional Case Management in Indiana is hiring RN Case Managers to conduct in-home nursing visits for former Nuclear Weapons Workers with chronic illnesses. You set your own schedule and provide direct care and case management.

Visits are weekly 1–2 hours, with vitals, home safety checks, and coordination of skilled and non-skilled caregivers. RN license, CPR, and home-care experience are required; BSN preferred.

Qualifications

• Graduate of a state approved school of professional registered nursing.
• BSN preferred.
• Current, unrestricted RN license in the state of practice (IN).
• Minimum of two years nursing experience including one year in home care or related field.
• One year supervisory/case management experience preferred.
• Current CPR certification.

Responsibilities

• Provide direct care and case management of assigned clients.
• Assist and collaborate with regional director to identify issues and improve services.
• Plan, implement and evaluate care; coordinate client care conferences as needed.
• Serve as an on-site clinical resource to ensure home care needs are met.
• Adhere to nursing delegation guidelines and agency policies.
• Participate in interviewing and supervision of clinical personnel as requested.
