Job summary

Professional Case Management is seeking RN Case Managers to perform in-home nursing visits for former Nuclear Weapons Workers in Indiana. You will conduct weekly assessments, manage medications, and oversee caregivers to meet patient needs in the home.

Qualifications

• Two years nursing experience including one year in home care or closely related field.
• Current, unrestricted RN license in the state(s) of practice.
• BSN preferred.
• CPR certification.
• One year of supervisory and/or case management experience preferred.

Responsibilities

• Provide direct care and case management of assigned clients.
• Assist and collaborate with the regional director and other personnel to identify and correct issues and/or improve services.
• Plan, implement, and evaluate care provided; participate in client care conferences as needed.
• Serve as a local on-site clinical resource and provide support to ensure client home care needs are met.
• Collaborate with staffing coordinators regarding staffing and scheduling of personnel within scope of practice.
